The Suffering of Others

posted by Julie O'Toole on May 8, 2015 at 10:04am

“With what incredible courage we are able to endure the suffering of others” -- My favorite quote from English garden writer Christopher Lloyd.  And nowhere does it apply more than in medicine.  And within the world of medicine, nowhere more than in the world of mental health.

When my neighbor is poor, he deserves it for his sloth and lack of thrift.  When I am poor, I am the victim of unfairness and persecution.  When a young man medicates his abdominal pain with narcotics, he just…

Intranasal Treatment for Anxiety

posted by Julie O'Toole on July 3, 2014 at 1:36am

Providers are finally beginning to dip our toes in the waters which have flowed from the realization that psychiatric and psychological disorders are brain disorders.  Case in point: social anxiety.

I don’t know whether we see quite a few eating disordered patients with social anxiety because these diagnoses frequently run together or because social anxiety is so prevalent in the general population, but we do.

Food phobia, the Kartini Clinic recipe

posted by Julie O'Toole on April 9, 2014 at 10:50pm

Talking to a mother whose son has just entered our Food Phobia treatment program I realized that I may not have done a very good job stating clearly what our “recipe” for success with this illness has been.  What can a parent expect?  What can a child look forward to?  What if a provider is forced to treat a child with food phobia in an outlying hospital; how might they proceed?

First, a bit about the diagnosis.

Food phobia is the term Kartini Clinic uses for the sudden onset of…

Mixed Symptoms

posted by Julie O'Toole on April 2, 2014 at 9:10pm

Recently I was making our daily rounds at Randall Children’s Hospital and received a lesson from a very young patient of ours.  I say it over and over: “your patients are your teachers”, and it is really true.  I was first told this by Dr. Mizuo Tottori, pediatrician and mentor to many other pediatricians in Hawaii. And life itself has borne out the truth of it, again and again.

This particular little patient has anorexia nervosa and has done very well.  Her parents are competent and…

The Evidence Is There, Now Make It Work

posted by Julie O'Toole on April 13, 2012 at 8:29pm

Jacques, a friend and parent advocate, sent me an online summary of an article discussing the current evidence base for the use of Olanzapine in anorexia nervosa.  Keep in mind as you read the summary that, regardless of the evidence base for efficacy and safety of Olanzapine in general and with anorexia nervosa in particular, this is still an off label use of this drug.  Off label means it is a drug approved for one condition which is used by physicians to treat a different…
